The Community Can Play a Vital Role in the Success of

Money Follows the Person

As of September 1, 2008, NCAAA became a proud partner in the Money Follows the Person Program (MFP). This statewide program is designed to maximize consumer choice and independence and to increase the use of home and community based services in lieu of the more costly institutional care.  The target population for this project is those individuals of any age who have been in residence in skilled nursing facility for at least six months. Outreach has already begun to identify individuals that might be interested in and eligible for transition to the community setting of their choice.

 

Though the program will cover costs associated with securing housing subsidies and other state-offered services, the actual cost of setting up a home for potential clients is not covered by the program.  Keeping in mind that since the Money Follows the Person consumer has been in the nursing home for a minimum of six months, these folks have little or nothing to return home to.  This lack of worldly possessions should not be a barrier to their independence.

 

Our regional partner in the project is Independence Unlimited (IU), and they have been engaged in nursing home transition activities for the past six years.  They have seen first-hand the necessity for fundraising and the incredible impact of even modest donations.  IU actively seeks the donation of household items for MFP consumers; please see a partial list below. As NCAAA lacks access to sufficient storage facilities we have chosen to focus our efforts on financial contributions. Therefore, NCAAA has established a regional “Freedom Fund” designed to accept and collect monetary donations to be used for the purchase of necessary household items for MFP consumers. 100% of your tax deductible donation will be used for consumers of the Money Follows the Person program and NCAAA will provide the necessary paperwork for your records.
